Abstract: Endometriosis is principally a gynaecological problem. However, it does frequently present to the surgeon on account of intra abdominal complications 1, 2, 3. Many general surgeons are unfamiliar with extra abdominal presentations that commonly pose a diagnostic problem. Therefore most general surgeons make the diagnosis post operatively from histology of biopsy specimens. 2,4. A case of long standing bilateral inguinal endometriosis presenting with bilateral painful groin masses thus mimicking groin hernias is hereby presented. Final diagnosis was made post-operatively from histology of excision biopsy specimen. The relevant literature is reviewed. Awareness and high index of suspicion as well as complete history and thorough physical examination are advised in the differential diagnosis of groin and other extra-abdominal masses with cyclical symptoms in women of the reproductive age.

Key words: Endometriosis, extra abdominal.
